VII.2.13.2 Place of Death - State
Place of Death - State is intended to collect information on the State
of death.
Coding Instructions:
Enter the abbreviation for the State in which the death occurred.

For cases diagnosed prior to
January 1, 2013:
VII.2.13 Death
Information
If the patient has died, enter the code for the state or country
where the death occurred in the Place of Death field. (The code for
California is 097. See Appendix
C, Appendix
D1, and Appendix
D2 for other codes.) If the patient is still alive, enter 997.
Hospitals are not required to complete the Cause
of Death field or DC (Death
Certificate) File No. field.
To report that a patient has died, make every attempt to find the
month and year of death. Approximations are acceptable when all attempts
to find the date of death have failed.
