Specification:

If Year of Diagnosis= 1988-2002:

If Type of Reporting Source = 6 (autopsy only), then Reason for no surgery must be 2 or 9.

If Year of Diagnosis = 2003 and forward:

If Type of Reporting Source = 6 (autopsy only), then Reason for no surgery must be 9.

For all years:

If Type of Reporting Source = 7 (death certificate only), then Reason for no surgery must be 9.

If Type of Reporting Source is 7 (death certificate case), then Date of Surgery Flag must be 10.
