034, 039, 046, 048, 051, 057 or 058:

BCR-ABL FISH cannot be blank

BCR-ABL FISH Date or BCR-ABL FISH Date Flag must be populated, but not both

For all other cases:

BCR-ABL FISH must be blank or 000 (not applicable)
