Message

If Chemo 4 NSC Number = 000000, Chemo 4 End Date Flag must be 11.

If Chemo 4 NSC Number = 999999, Chemo 4 End Date Flag must be 10, or blank

If Chemo 4 NSC Number = [value], Chemo 4 End Date Flag must be 12, 15, or blank.

Specification

This edit is skipped if any of the following conditions are true:

  1. Year of Date of Diagnosis is not 2011

  2. Case is not one of the following site/histologies

    1. Breast (Primary Site = C500-C509 and Histologic Type ICD-O-3 = 8000-9044, 9060-9136, or 9141-9582)

    2. Colorectal (Primary Site = C180-C189, C199, or C209 and Histologic Type ICD-O-3 = 8000-9044, 9060-9136, or 9141-9582)

    3. Chronic Myeloid Leukemia (Primary Site = C421 and Histologic Type ICD-O-3 = 9863, 9875, 9876, 9945, or 9946)

  3. If County of DX <> 002, 003, 005, 009, 029, 031, 034, 039, 046, 048, 051, 057, or 058.

  4. This edit is skipped if Chemo 4 NSC Number is blank.

If Chemo 4 NSC Number = 000000 (Chemotherapy was not planned to be administered OR no additional chemotherapy agents were planned), then Chemo 4 End Date Flag must = 11 (no chemotherapy agent administered).

If Chemo 4 NSC Number = 999999 (Unknown if chemotherapy therapy planned OR not required for this primary site/histology), then Chemo 4 End Date Flag must = 10 (unknown if any chemotherapy agent administered), or blank

If Chemo 4 NSC Number not = 000000 or 999999, then Chemo 4 End Date Flag must = 12 (chemotherapy administered but date is unknown), 15 (chemotherapy is planned as part of the first course of therapy, but had not been started at the time of the most recent follow up), or blank (a valid date was provided).

8/2011

This item added for 2011 as part of the CER project.

11/2/11

Deleted logic allowing only date flags of 12,15, or blank for actual (not 000000 or 999999) Chemo 4 NSC Number. (Chemo 4 NSC Number refers to chemotherapy agents administered OR planned.)

Temporary adjustment to the CER edit to allow for blank flag values if NSC Number is 999999.  This edit will revert back with Eureka 10.2.

Added the following information back into edit, after release of Eureka 11.0.

  • If Chemo 4 NSC Number = [value], Chemo 4 End Date Flag must be 12, 15, or blank.

  • If Chemo 4 NSC Number not = 000000 or 999999, then Chemo 4 End Date Flag must = 12 (chemotherapy administered but date is unknown), 15 (chemotherapy is planned as part of the first course of therapy, but had not been started at the time of the most recent follow up), or blank (a valid date was provided).
