Message:

IF 614:  If Type of Reporting Source is [value], Date of Diagnosis [value] must be the same as Date of Last Contact [value]

Specification:

If Type of Reporting Source = 6, then:

Date of Diagnosis = Date of Last Contact and Date of Last Contact must be full CCYYMMDD dates and the dates must be the same; else (Err #614)

If Type of Reporting Source = 7, then:

Date of Diagnosis = Date of Last Contact and Date of Last Contact must be full CCYYMMDD dates and the dates must be the same; else (Err #614)

Historical Changes:

2010 Data Item Changes:  Updated IF 614 so Date of Last Contact cannot be blank, zero or unknown. CCR names (Date DX; Date Last PAT FU; Report Source) changed to NAACCR names.
